Building a Platform for Precision Anesthesia in the Geriatric Surgical Patient
The research team is creating a foundational infrastructure in order to develop a precision medicine approach for geriatric patients who require surgery with anesthesia. The team plans to build the first of its kind comprehensive database of demographic and risk factor questionnaire responses, biobanked blood specimens, intraoperative electroencephalography (EEG), and inclusive cognitive testing throughout patient interaction starting at the preop appointment until a year later. The team will test the feasibility of collecting large-scale comprehensive data in the geriatric surgical setting and thus set the precedent for subsequent multidisciplinary studies. The team plans to develop an analysis platform with a suite of statistical tools capable of exploring this multidimensional dataset. This will include preliminary analyses of raw data including processed EEG measures and deep immune profiling using mass cytometry. Layered upon this preliminary analyses the team will build innovative data mining machine learning tools to identify predominant features and interactions contributing to PNCD pathology. These novel tools address pragmatic challenges faced by many pilot clinical research studies. The team anticipates a suite of tools will be useful to identify subgroups of patients within a featured space and with which proposed targeted therapeutic interventions can be made. The platform will allow the testing of interactions and thus provide new outcome measures and a more comprehensive view of pathophysiological mechanisms. The multidisciplinary research outlined will create the infrastructure to support an imperative and significant first step in developing precision geriatric anesthesia care. This project launches geriatric surgical and anesthetic care into the era of big data and artificial intelligence in healthcare. Completion of this work will provide the platform to build custom therapeutic strategies from a comprehensive PNCD model.
